An older woman, Charlotte (Colleen Camp), who has been taking care of her husband Eddie (Meat Loaf) for the last six months, is rushed to Princeton-Plainsboro for treatment after collapsing at Eddie's deathbed from respiratory failure. The case is further complicated as Eddie's condition improves while Charlotte's deteriorates. Meanwhile, the team tries to deal with their grief over Kutner's sudden suicide. When a priest (Jimmi Simpson) who runs a homeless shelter sees a bleeding Jesus hovering at his doorstep, he is admitted to the ER. House takes on the case as a distraction for the team while he confronts Foreman and Thirteen about their relationship.

Thirteen takes a woman (Angela Gots) with whom she had a one-night stand to Princeton-Plainsboro after the woman has a seizure in Thirteen's apartment. House and the team take on her case, House using it as an opportunity to explore Thirteen's personal life. Thirteen ends up finding out the patient only slept with her to get to House, who had been rejecting the woman as a patient for many years.

House orders a pain assessment to rule out anything psychosomatic, and an environmental scan. House and the team take on the case of Nick Greenwald (Jay Karnes), a book editor who loses his inhibitions and starts insulting co-workers at a dinner party one night before falling ill. The team realizes Nick has frontal lobe disinhibition, which has caused him to lose his filter and vocalize all of his innermost thoughts, resulting in extremely insulting outbursts. As Nick's free speech leads to some amusing and insightful comments regarding the team, he must deal with the consequences of being unable to lie to his wife and everyone else important in his life. Meanwhile, House suspects Wilson and Taub are keeping something from him when he catches them both in a lie.

The team resists, with Thirteen asking how he'd like to be cut off from Vicodin. Foreman once again tries to talk Thirteen into having a relationship. Kutner and Taub come in with the results which show that there's air near the patient's intestines. Kutner and Thirteen decide to do an angioplasty to find the blockages. House comes in to find his team believing they have solved the problem. However, House realizes the patient tried to inject air into a blood vessel to kill himself - the air they found wasn't a symptom.
The responses of the patient, wife and son are consistent. Foreman does the environmental scan with Thirteen and find more drugs than House uses. Foreman thinks Thirteen's avoiding him after they kissed. Thirteen makes it clear she doesn't want to drag down Foreman. They find quail and Thirteen thinks it's food poisoning but Taub thinks the profile shows the pain's psychosomatic. House thinks it's rhabdomyolysis from food poisoning and orders a biopsy of the patient's muscles.

Cuddy tries to adopt, but after House tries to convince her not to, the adoption falls through anyway. When House tries to show his condolences and Cuddy snaps at him for changing his tune, they embrace and kiss. Wilson realizes that the two are very attracted to each other and attempts to make them confront their feelings. We soon begin to realize that although they are both reluctant to begin a romantic relationship, both want the other to be more involved in their lives. However, the biggest change in Season 5 was the departure of Kal Penn from the series so he could take a position at the White House.
An agoraphobic man (Todd Louiso) falls ill and refuses to leave his home to be treated at the hospital. Therefore, House and the team go to his home to figure out what might be wrong. As a result, Foreman begins first to get concerned about her behavior, then starts to feel protective. It soon blooms into a romance, but House feels it is affecting their medical judgement and tries to make them break up. Wilson slowly starts to recover from Amber's untimely death, but begins to reevaluate his life and to contemplate resigning from his post at the hospital. House's original regret over his role in Amber's death seems to have worn off as instead of being supportive, he merely tries to convince Wilson that he is overreacting to the situation. The fifth season of House premiered on September 16, 2008 and ended on May 11, 2009.
House is coerced into going to his father's funeral despite his protests. His team takes on the case of a young woman (Samantha Quan) who collapsed while in China searching for her birth parents. House works with the team via cell phone, while Wilson drives him—reluctantly—to the funeral.
